It sounded like 80% of the problem was that John was dealing with a
programmer who was satisfied with an approach based on a erroneous
understanding of COBOL semantics and code that gave the appearance of
working some of the time while containing known semantic errors that
guaranteed unpredictable failures.

That is a personnel problem.  Perhaps the individual is inadequately
trained or just mentally unsuited to the rigors of programming.  The
ultimate solution (attitude change and better training, or replacement)
may not be under John's control.
