I see the same at PDF pages 47 and 48, document page numbers 11 and 12. We also need to get John some help in creating a PDF Table of Contents (as well as a complete Index) with hyperlinked page numbers so you can go directly to the document page from the TOC (or from the Index). That would be extremely useful in such a large document.
I don't know how to do that myself, but I'm sure the expertise is out here somewhere. Volunteers?
