Ok, totally tacky, following up on my own message, but I got my problem solved, after banging my head against a co-worker for a few minutes.
It turns out having files lying around on your 191 from old second-level installs (like 5.2, 5.1, ... *cough* ...4.1) plays havoc with this whole process. INSTPLAN is not picky about which filemode $INST$ $FILE$ is on, and found a copy from our 5.2 upgrade in filemode A to read and update. INSTDVD EXEC is picky and expects it to be on filemode C, where it had been untouched by INSTPLAN. I started over with a clean 191 and it works (is working now) great That was a fun one.
