Two things:

1) make sure you followed all the steps in the program directory. The  
PDF panels and code are not in the default ISPF libraries, and you  
need to modify the ISPF startup exec to add the additional Txtlib and  
maclibs.

2) make sure the ISPF saved segment is not overlapping something else  
(like CMSPIPE). The install instructions for ispf and PDF are ancient,  
and a lot of things have moved around in memory since then. The segmap  
tools on maint can give you a map of where stuff loads and you can  
read the Vm Service manual to find out how to move the segment so it  
doesn't conflict.

My first guess would be #1.
