OK, here's hoping it is 3 times lucky and this is not a stupid question :-)
 I've been digging through it for about 90 minutes now and am finally
sending this off.

I'm not getting alerts, and when I dig into log files and the "Event Log"
at the bottom left of the old Nagios interface, I see a few things warning
about the alert timing out :

e.g.

And of course nothing in the mail logs either.

But then I extract that long "printf | /bin/mail" command out of there
verbatim and paste it into a shell on the icinga server where I am logged
in as the icinga user, and it works with no apparent delay to make it time
out.

So what do you figure is causing the time out?   I know this machine has a
bit of a tricky DNS configuration that needs fixing and I know that this
can cause mail issues, but other cron emails and similar things seem to be
working.  Though (and this may answer my own question) the "mailq" command
on the icinga server takes some 45 to 60 seconds to run even when the queue
is empty (which it almost always is)

Would I be correct that whatever is causing "mailq" such headaches, is also
causing the icinga notifications to time out?
