Actually, bandwidth USAGE is frequently charged for in many parts of the
world. In the US, it is common for small businesses to have flat rates or
included bytes sufficient to make the usage flat rate. Major last mile
providers also change a lot more when provisioning for static IPs and
presumed higher bandwidth consumption.

Every colocated or shared server agreement I've investigated recently has
included bandwidth charges for exceeding the included usage. That applies
in spades to folks to provide datacenter cages.

In some parts of the world, even dialup customers have bandwidth changes.
Data costs in some third world countries can be many times that of the US.
