Bandwidth is the main thing to be considered.  Putting this of the top 
of my head - for A/V each "client" you will need about 256Kbps 
upstream/downstream.  The server will have to support the stream for 
all the conference clients - the b/w requirement grows.

I am not sure if any commercial hosting will support such an 
application.
