Standards. But it is a combination of all the above (1-5)

I hope, you are referring SCSI Controller cards. Today in Low-end
mother boards, IDE and SATA controllers are in-built. In high-end
mother boards, SCSI may-be in-built. If it is not in-built in the 
MB, you use PCI controller card to interface with SCSI HDD.
