well, it has been quite convincingly argued that premature over optimisation will dig you a deeper hole than almost no optimisation :) but sticking with common sense and knowing what you are doing, or reaching out to people who would, is always a good thing

No need to get into all that. Just plan for scaling. This means that
you should already know how to do load balancing and HTTP redirection
and take care of session persistence.

I am not sure the question was so much about the scale of youtube's operations as much as the content side of things....

There are a few open source apps that try and give you a media portal, the sorts that would be welcome in an intranet. Out in the open its mostly a case of get the tools and components and build your own framework around it
