The earliest referenced instance of the term is http://web.archive.org/web/20071212203835/http:/www.heise.de/ct/english/98/12/230/ PHP for a long time was inherently tied to MySQL as well. I would note that LAMP became a evolving acronym where "P" was substituted with various languages over time however the combination of Linux, Apache, MySQL and PHP as LAMP was historically a important marketing and development initiative by Oreilly, MySQL DB and others that helped propel Linux in its early days.
