Sorry for the delayed response.
The software vendor is Insta Hospital Management Solutions.

The software is not open source. However it has been deployed on a
Ubuntu 8.04 server, which has not been updated. Although they do give
root access to the system. Which was how I was able to these details.
I didn't update the system either lest some dependencies should fail
to work.
My blog post regarding the same is getting ready. Will be out soon.
