Yes. Of course it is necessary to acknowledge the original source and
distribute the part of the content under the same license.
Modifications should be under same CC-by-SA license.
Applies to all versions of the license.

Other parts of the book can be under a different license.

No. That would be violation of CC-SA and the matter can be taken to court.
