Ankur, there is an error in your Mailscanner.conf.
%org-name% should be a single word. Else the headers become invalid. Like the X-Crano... one above.
We use squid 2.5 stable under RH9 in the office. Sometimes we have developers working on sites that do not have DNS records but only IPs. We do have to check the working under the dns record to be. I added the IP and the name to the HOSTS file on the server now it is able to atleast go to the right server. Everything is redirecting to the server via the domain name , except the web requests. Those throw up an error "(104) Connection reset by peer" . I tried searching in Google but could not figure out what the problem is. It seems Squid is doing its own name resolution without actually looking into the hosts file or is disregarding the HOSTS file record if it finds nothing in the DNS for the same name.
The error message says that a host had been contacted but that host refused a connection. Squid does have some dns helper programs which sometimes does pretty developer-unfriendly things like DNS caching etc.
Have you tried using a packet sniffer to find what is going on?
