Bingo! Thanks, Ashish. It was the '32 GB Clip
problem'. 

I had inserted an extra jumper (the 32 GB Clip) to set
my HDD to be the Primary Master. Apparently, the label
on the disk had put up the jumper diagram just below
the title (set limit to 32 GB) to set the HDD to
master, and I failed to notice the other diagram which
was to set it as master, without limiting the HDD to
32 GB. 

Thanks to all those who read that long email and/or
offered suggestions. 

Here's an interesting forum thread related to this
