If the fan isn't running (it's not excessively loud, but there should 
be no doubt) get a replacement.  The rev.B fan will be inactive under 
only two circumstances...  1) your iMac is turned off; or 2) the fan is 
faulty.

It may simply be that the fan has accumulated enough spooge to prevent 
it from starting (it's a very low torque device) when power is applied. 
  Unless you can give it a proper cleaning, replacement is the 
preferable option.
