This happens infrequently on our iMac flatpanel. We fix it by unplugging each of the 3 USB connectors, including the keyboard/mouse and then plugging them back in different slots and the red light comes on again.
I don't like it; it shouldn't happen...but the fix always works. Dunno why.

As its a hardware problem it will effect both OSX and 9.x, any system. If the mouse fails at startup in OS9 the mouse pointer is typically frozen in the top left hand corner. When it dies in use, often there is no red light when the mouse is turned over.
