Look at the header. Possibly something in the header
like an IP address or something. I put certain IP blocks
in occasionally myself that refuse to do anything about
their SPAMmers.

Subject: [IMail Forum] Spam ... or not Spam ... that is the question

I have anti-spam rules setup here. However, for some reason, I continue to
get messages sent to my spam folder that don't seem to relate to any of my
rules! All seem to be messages with attachments. One, which was sent to my
spam box on many occasions, had an attachment of a very young, over-weight
baby sitting beside a wrinkled puppy! This message was in my spam box many
times. Nothing at all in the message containing my rules. I found another
today with an attachment. The attachment was a small, two line note in .jpg
format, but contains nothing we consider spam by our rules. What the heck
would be causing this?
