Make sure that IUSR_MachineName and/or IWAM_MachineName (depends on if you
are in or out of process) have appropriate permissions to write to the
database table.

You didn't mention if you were on NT 4 or Win 2K or if FP 2K was involved.
There are other headaches with FP 2K and Win 2K with the accounts FP sets
up.
