We do it with Postfix on a Linux box. That also lets us put ASSP in front of it which although I've heard some grumbling here about it is a great antispam frontend. This postfix/ASSP frontend sits in front of our Imail box and also in front of our Exchange box. We've even used it to control major spam bombs against a few of our customers on occasion by temporarily redirecting their MX records to us until the bomb subsides.
The other day I got an email from a user complaining about the huge increase in spam messages he was getting. I started looking into it and didn't see any issues. I called him back and asked for more information. He was all grouchy because he had gotten 3 spams. Stifling my laughter I politely told him I'd review the system further and ended the call. Needless to say I'm very happy with the Postfix/ASSP solution.
