I had the same type of problem awhile back with one of my ETRN customers.
Upon investigation, we found that, although it was properly dialing up at
set intervals, their server(Exchange) hadn't been properly configured to
send the ETRN command. Thus, they were only getting mails that came in, or
got resent from queue, while they were dialed in.
