Have you tried setting up a virtual domain using the standard templates to
see if the problem still exists?
When I have an occasional problem like this, I set up a mock domain, assign
it an internal IP, and see if I can duplicate the problem with the standard
templates. Then if the problem isn't duplicated, I know I screwed up
something in the templates and the hunt is on.
By mock domain, I use foo.com with an internal (192.168.0.X) IP address.
Make a web directory and load the default templates.
