Dealing with just the first part of your query: "How to to determine why an
unexpected message was caught in a filter?"

        I am using Outlook 98 as my email client, and using IMail Global Rules to
filter messages with potential virus-laden attachments to a "quarantine"
folder in an IMAP account called "virus."  With Outlook as one's email
client, it's useful to use an IMAP account as the destination for filtered
messages  in order to view the filtered output with a text editor.  If a
message is  downloaded to a POP3 client with Outlook, it will be downloaded
to your local machine of course, and embedded into a larger single .pst
file, rendering it more difficult to examine.
        When an unexpected redirection occurs, I "move" the IMAP message, using my
email client (Outlook 98) from the quarantine folder to another (empty)
folder within the IMAP virus account - the folder is called "Test".  This is
intended to isolate the file, making it easier to examine.  This folder, on
my IMail server, will be listed as the file, (using UNC
convention)\\IMailserver\IMail\users\virus\Test.mbx

I can scan this .mbx file using notepad or an equivalent text editor and
search for the strings I was hoping to filter.  Viewing an .mbx (mailbox)
file with a text editor allows you to view an email message directly in its
entirety - Header, body and attachment.  If the string (scr, vbs, shs, exe,
etc.) turns up in an unexpected location in the email message (i.e., in a
position other than in the attachment), I know that my filter will need to
be adjusted.
