You can't use both. If you use just "Relay for local hosts", you're allowing spammers
to send mail through your server, but only if they are willing to use a (likely fake)
return address on your domain.
You want to use "Relay for Addresses", which lets you restrict to just your local IP
ranges.

I believe the accept.txt will prevent other SMTP servers from sending mail to your
server (maybe someone can confirm this?). That's a bad thing.

Not at all. If everyone locked down their mail servers, the total amount of spam sent
would likely go down dramatically.
