That's a problem we had on one of our domains.  A spammer was using about 8 
different fake usernames on a domain we have (and also using the same 8 
fake usernames on a lot of other different domain names).

No, there isn't any (easy) way to detect this.

No, that's impossible.  All you get is a bounce message.  It is up to the 
server that was relaying the message to include the IP address, but most 
will not.  You can get the IP address of the mail server that is relaying 
the spam, but probably not the IP address of the spammer.

What we did is treated it as abuse of our domain.  We thought legal action 
would be trivial and unnecessary (even though it was going on for almost 6 
months).  So instead we kept track of all the information about the 
spammer, and finally got to one of their web sites, clicked on the "order" 
button to find the company that processes their credit card transactions 
and their account number.  We politely explained to the credit card company 
what was going on.  They replied that they terminated the account -- and 
guess what?  No more spam.
