First thing I would check is the password for the SA user.  Default is no
password and it is often not changed after the initial install.(Or it is set
to an easy hack.)  After that it depends on whether you are using NT
authentication or SQL authentication.  I always turn on auditing and log all
successful and failed logons.  Surprising what you find.

There may be some patches that need to be installed.  The easiest way is to
check is with new Security CD from Microsoft.  However, I have found that
most problems result from a User/Password or permissions hole.
