Technically, HTML is part of MIME segments, not attachments. With MIME,
it's treated the same way as text segments. A good rule of thumb for
distinguishing a MIME segment versus an attachment is that attachments have
names ("program.doc" for example), whereas MIME segments normally do
not. You are correct that a lot of spam does use HTML. However, blocking
HTML MIME segments would block about 1/2 of all legitimate mail, so it
would not be recommended in any case.
