It's actually very simple...assuming you're using an external database.

- Make the form in html
- Insert values into database with language of your choice (asp, php, perl,
C#, whatever)

If you're using the iMail database (registry) it's a bit different...you'll
need to write a component to update the registry since you don't have direct
access to it from any web language (except maybe C#...not sure)
