What would cause IMail to randomly reject emails? I have a user who is constantly complaining that he cannot send to certain domains.

Is IMail rejecting the E-mail (incoming mail to the user), or is the remote mailserver rejecting the E-mail (outgoing mail)?

It sounds like you're talking about outgoing E-mail, in which case the problem would be that the remote mailserver is rejecting the E-mail. Have you checked your log files to see what happens when those mails are sent (look for the "SMTP-" or "SMTP" (not "SMTPD") lines after the "RCPT TO:" line, and you should find the problem).

Coincidentally, certain domains cannot send to him. I checked ORDB.ORG and we are not on the list. What can cause this?
