Open a command prompt, cd to your spool directory and type:

find "ERR 005" sys????.txt
(replace the question marks with the current log, or a previous days log)

See if there are a lot of lines returned, complaining of an ERR 005 thread
exception handled.  We have been having a similar problem, although ours is
intermittent and the ERR 005 logs are the indicator we use.  When it starts
happening we can stop and restart queue manager and the spool gets cleared
and mail starts delivering for  a while.  We have an open issue with
IPSwitch about this now but we are currently waiting a solid fix.  Most
recent was the 8.03 patch which didn't give us immediate results but soon
after it stopped again and has been running fine for a while now.  Prior to
8.03 we had to resort to running a batch file every 20 minutes to restart
the queue manager process to make sure mail kept moving.

Have you modified any of the values in queue manager (delivery threads,
retry threads,listen pipes, etc)?  I played with those once and caused the
same thing but it was fixed by returning to default values.
