Check your logs, they will tell you exactly what happened and why. My guess...you have another rule that comes before the one you are talking about. It matched that rule and then it was done. Rules will run until it matches the first one and then it's done.
Seriously though..your logs will have all of the answers you seek. Look for the transaction regarding this e-mail and it will tell you. Always check your logs first before posting.
