Ah...next time might want to be a little more clear in your questions...
 
See if this helps.  It's for SQL Server, but you should be able to adapt it relatively easily.
 
 
I seem to remember that once you've specified a host as using an external database, the tables get created automatically.  Adapting the above should allow you to form a connection to your mySQL box.
