I am actually using both right now.  We use the SMS SMTP as a gateway
for all incoming mail from the outside world and all outgoing mail to
the outside.  We are actually running two internal mail servers an
Exchange server and Imail.  We started out with just the gateway in
place but when are Exchange server started finding viruses that the
gateway product had missed and our end users on the Imail system found a
few viruses with their desktop scanning software we decided to add the
additional layer of the Symantec engine within Imail.  Since
implementing it our clients have not found any more viruses, but the
Symantec engine has only found four viruses on the server.  The Gateway
product detects and removes around 150 viruses a day.  One nice thing
that Symantec has added was the ability to drop mass mailer viruses,
instead of responding to them or notifying about them, this allows us to
still notify on viruses that require it, like word or excel macro
viruses.  We are also using the gateway to reduce spam with decent
results.  It blocks about 58% of all messages delivered to us by using
RBL and Hueristic scanning.  There is a low rate of false positives that
can be managed by whitelists.

It  scans  all  mail,  i.e.  all  mail  processed  by the local/remote
delivery agent.

POP3  and IMAP4 are neither incoming nor outgoing in common mailserver
parlance. They are mail client protocols.
