We monitor our customers religiously to avoid any one customer jeopardizing
service for everyone else.  We also turn potential customers down all of the
time because their business would violate our terms of service.

Reports are run every night and inspected every morning to let us know of
any potential abuse.  If we see it, we jump on it right away....usually
within an hour or two, but always within a few hours.

So far everyone has understood and cooperated, but we will shut abusing
customers down if they do not stop the violation immediately.

This might be part of it. It is often not possible to know. Hosting
services also quite frequently use shared hosting - which means that
much of the time many web sites will be "taxed" for no good reason.
All of these arguments have already been made.
