Using LDAP? Our admin tool would "lockup" when we tried to add a user. Mods sometimes worked, sometimes reverted when we hit apply, or the admin tool would terminate. Other symptom was slapd would sometimes peg at 25-80%.
Our issue was LDAP db was corrupt. I'm in the process of rebuilding it currently.
