The customer deleted the account in Outlook and recreated it, and is now problem-free. Go figure.

What is the response to the EHLO command after you get the banner...there should be 2 AUTH lines listed as capabilities. If they are not there go to the advanced tab of the SMTP service and uncheck the disable SMTP Auth reporting. Bounce the service and try it again.
