My company does gateway spam and virus blocking for folks like yourself. We service clients all the way down to 3 addresses, and it is much more economical for smaller and even medium-sized installations to outsource when you consider both the cost and labor involved in managing this for your own server, and of course those of us that do it as a business should be able to achieve much better results than you can on your own. We generally block 99.8% of spam with a 0.02% false positive rate, and that's pretty much the theoretical limit. The added benefit of having a redundant gateway is also beneficial as it will spool your messages when your server or Internet connection is down.
I don't want to come off as too pushy in promoting our service on this list, so let me also mention that there are many others like myself that offer such services and aren't concerned with taking on very small clients. Most of us are Declude based systems around here, but it takes a lot of extra effort and understanding to reach spam block rates above 95% with a high degree of accuracy.

There are just a few of us on this server. It doesn't need to be perfect. The only requirement is that I don't want to install perl on the server. I don't know anything about it, and can't afford the time to learn about it (hopefully I won't get flamed for that requirement ;-) ).
