It's not exactly list functionality, since members can't send to each other, but we use a newsletter management system we built a few years ago. It sends individual messages to each user with an unsubscribe link in it, and has targeting capabilities. The sign-up process sends a confirmation email, then a welcome email after confirmation. Newsletters can also be scheduled in advance.
It could, however, act like a normal list with a little tweaking through program aliases that check to see if a user is allowed to send, and then queue the message for normal delivery. It relies heavily on a SQL backend.
