Yes it is mailbox @ domain not mailbox @ host. Some mail servers will try to deliver to the A record as a last resort.

No, what comes after the @ tells the mail server what domain to look at for the MX record. No short cut just the way SMTP interacts with DNS.

How I am searching:
Searching for mail.salemradiology.com MX record at i.root-servers.net [192.36.148.17]: Got referral to J.GTLD-SERVERS.NET. [took 7 ms]
Searching for mail.salemradiology.com MX record at J.GTLD-SERVERS.NET. [192.48.79.30]: Got referral to bcw4.bcwebhost.net. [took 196 ms]
Searching for mail.salemradiology.com MX record at bcw4.bcwebhost.net. [66.224.41.3]: Reports smtpgw.slmdc.pnwtg.com. [took 116 ms]

Answer:


Domain Type Class TTL Answer
mail.salemradiology.com. MX IN 43200 smtpgw.slmdc.pnwtg.com. [Preference = 10]
smtpgw.slmdc.pnwtg.com. A IN 68059 66.224.124.100

Note: the first row in the table, it looks at the domain for the MX record. The domain follows the @ in the email address.
