OK, thanks for the update. It can be very frustrating to debug these types of issues, since there are so many variables! But eventually the finger-pointing stops and the problem-starting happens... ;-)

For the record, here is the /etc/pam.d/imap file that is installed on my Mac at home:

# login: auth account password session
auth       sufficient     pam_securityserver.so
auth       required       pam_deny.so
account    required       pam_permit.so
password   required       pam_deny.so
session    required       pam_permit.so
