We see problems like this occasionally. we are running imap-2002-RC8 with some patches from an older debian wu-imap package that enable Maildir support. debian has seen the light and no longer distributes these patches, but we still use them due to avoiding the pain of conversion. We'll be fixing that in the not too distant future, I hope (it wasn't my decision to go with Maildir in the first place but it has never been at the top of the list of things to fix).
One thing I've been able to determine is that when this happens, it is often related to a bad .uidvalidity file in the Maildir directory, which happens occasionally when our mail server's raid card flakes out. (can you tell, this is an old server in need of an upgrade?)
