We don't run IMail.  We're all Linux - running Postfix, Courier, maildrop,
amavis-new, spamassassin on 5 servers.  Two of the servers are load balanced
gateways (using IMGate filtering techniques); two are backend mailbox
servers which also perform load balanced pop/imap; and one is an NFS server.
The 4 non-NFS machines all have access to the same user database.

Anybody that has Outlook set to "send immediately" may easily reach 10 =
SMTP
AUTH sessions in 20 minutes.  I've got the anvil rate_limit set really =
high
right now until I figure out what to do with it (100/60s).

anvil should interfere with SMTP AUTH, unless a user's mail program is =
SMTP
AUTHing  10 times in 20 minutes.

but if that's your case, then, "man smtpd"

        smtpd_client_connection_limit_exceptions
               Hostnames,  .domain  names  and/or  network address
               blocks of clients that are excluded from connection
               count or rate limits
