Andrew, thats a good suggestion. Yes the form has abit of some issues but I think it is a dummy on which Bukuru can build because I see some duplicate values in it. I believe as the OpenMRS Team, we can work out a way of having a standadized form for Tanzania which is more user friendly. Thanks Bukuru for the work. It is great.
