Hi Joseph,
You need to make sure you define a Program and Program Workflow to use
within the module. This is one of the few things that is baked into the
module which is specific to how our implementation wanted it which could
be made optional if someone were willing to invest some time to do so.
If you are just evaluating this module to see how it operates, you could
just create a dummy program and dummy program workflow (I think you'll
need to make sure you pick concepts of the appropriate type to associate
with these), and then configure these in your global properties. This
should fix your problem.

I had never seen that Location selector widget before. I just
reproduced your problem by going into my Administration -> View
Location Hierarchy and then changing the Location Widget Style
from "Simple" to "Tree". So this appears to be an issue that only
arises if you are using the "Tree" style Location widget.
This is _probably_ something that could be fixed in the module,
but I would need to look into it in more detail. However, my
guess is that the real place this needs to be fixed is in core, as
core needs to ensure the Location Widget options it provides are
compatible with one another.
Your best short-term solution is to use the "Simple" style for
your Location widgets, particularly if you are just evaluating
this module for suitability. If this is not an option for you,
then I would raise this issue on the developers list and create a
Trunk and/or a Simple Lab Entry module ticket, and advocate for
getting bug fixes applied.

Location for the Order. I just tried this out on an OpenMRS
standalone running 1.8.2 and it added the order fine. Did
you choose a location from the Health Center dropdown? You
cannot leave this blank.
