Subject:  Update on Insect and 'Biting Pests' repellants




Hi,
 Had previously written in August 2007:

As a Consultant Dermatologist in private practise for many years and having
traversed the four corners of India, including the war zone [read
leech-infested areas]; I would like to offer a few words of advice.
-         Some bites especially Soft tick [Argasid] bites result in
formation of a granuloma [a small nodule] which may itch for many months to
years. They may remain quiescent for a long period of time, only to flare-up
on being bitten again by the same or another species of ‘biting pests’. It
is important to note here that these lesions do not readily respond to
regular treatment even with topical super-potent steroids [eg.Halobetasol,
Clobetasol] and oral antihistaminics [eg.Polaramine, Atarax, Avil]; hence it
is advisable to take a short course of systemic steroids [eg.Wysolone] in a
staggered dose. This can be prescribed by your local G.P.
-         Insect repellent sprays containing Diethyltoluamide [Deet] eg.
Johnsons’ Deep Woods OFF, are effective but not available in India.
-         My personal choice is Permethrin available as an anti-scabetic
[Permite,HHMITE] or an anti-lice preparation [Perlice]. It is affective
against most biting pests and has the added advantage of having a residual
staying effect on the skin. On my recommendation it is now being used by
most B.N.H.S. members during camps. It can be applied directly on exposed /
covered areas of the body including the face. Leech guards should be soaked
in it and dried before use. Ditto for shoes and mosquito nets.
-         And finally, no outdoor protection is complete without adequate
sun protection. This could be in the form of a cap, a hat with a 3 inch
brim, a long-sleeved shirt and a sunscreen with a Sun Protective Factor
[SPF] greater than 15 for Indian skin, preferably waterproof.


Would now like to add:

     For those venturing out into the wild, additional protection can now be
obtained by bathing with a soap containing Permethrin [available as HHMITE
soap or PSCAB soap]. Clothes can also be [incompletely] rinsed using the
same soap.
