1. Icon filenya harus dengan format gif, tidak bisa hanya sekedar rename file. Informasi lokasi dari icon (*.gif) file nya ada di %ORACLE_HOME% \forms90\java\oracle\forms\registry\registry.dat --> berupa VIRTUAL PATH bukan PHYSICAL PATH, dan tidak menggunakan registry di windows (regedit).
