Is it possible to add custom fields to the repository?

No. But CVS is open source so you can extend it yourself if you are inclined.

I want to attach some meta data (eg: description, synopsis etc) to files in
the repository,

If you want to keep metadata about files you can add a file (metadata.txt or something) that and keep the information in there. It's not ideal but periodic review can catch problems.

Secondly, can the revision field by a string or does it need to be an
auto-incrementing number?

Don't mess with the revision number (1.n). That's the property of CVS.

Use tags (cvs tag). They can be strings, subject to certain restrictions.
