OK - version 2.2 of CFX_iMSMail now adds the Content-disposition header.
Also, the tag has supported user-defined content types for quite a while.
What you need to do is:

<cfx_imsmail ... attachments="test.doc|application/msword" ...>

And then the content type will be filled in.  You can also list more than
one attachment like:

attachments="test.doc|application/msword, test.xls|application/msexcel"
