The tag will do that if the file is locked.  This happens often with IIS.
What can happen is that the user views a mail and the attachments are
created.  If the user views an attachment, IIS locks the file for a default
of 60 seconds.  If the user views the mail again (or another mail that has
the same attachment name) then the tag will see tha the file is locked and
increment the name.
