I ran into this yesterday doing a SXCEb72 CD install. The drive was  
either flakey or weird in some way Solaris didn't care for. See if  
your install media is still accessible after you get dumped to a shell?

Some research also suggested that it may occur with certain  
(unsupported-ish?) SATA controllers:
