Hi - It looks like you may be running into a problem that I ran into when I was running some tests on a Dell. For some reason, on some systems, the device info is not provided correctly and is not reported correctly in /sbin/biosdev. The device isn't recognized and the lucreate process is unable to create the boot environment. There is a bug filed against this behavior.
